In this the applicant woman has to give brief information about her annual income. The annual income of the family of a woman applying for Ladaki Bahin Yojana must be less than two and a half lakh rupees. Women whose income is more than 2.5 lakh rupees are ineligible for this scheme.
In this form, the woman should fill her personal information like name, husband’s name, mobile number and other information. Information about income from agriculture, business and other means should be given. After that you should sign yourself. The woman needs to paste her passport photo on the form.

- The female applicant should enter her name first. After that, if married, enter husband’s name, if unmarried, enter father’s name.
- The woman should fill the form accurately with her own age, occupation and complete address of the place where she is currently living.
- After that the means of income like agriculture, industry and other annual income should be mentioned in the application.
